Patient Information
At Chatham Heart Center, we want to provide you with the best possible cardiovascular care. In order to do this we do not want to duplicate tests nor medications.
We ask you to bring:
- All your pill bottles to each visit. This ensures changes have not been made as well as helps us prevent medication interactions.
- Any and all lab results and tests from other doctors.
- Your insurance cards to each visit.
- A government issued photo ID.
